The three submissions below are required annually for all AMA affiliated collegiate chapters. Each year these three reports must be received by the deadlines below (if being judged for competition or not) in order to remain chartered.

Submittal Process:
Chapter Plans and Annual Reports must be uploaded using the Chapter Reports Portal.
Email submissions will no longer be accepted.
Only Chapter President, Second Contact, and Faculty Advisor, reported by the chapter through the Online Officer Report Form are approved to upload through the Chapter Reports Portal.
